How CarePath RPM Works

1. Patient assigned Bluetooth devices auto-pair with the CarePath Connect Edge device, which transmits device data to the cloud-based CarePath platform.
2. The Patient’s Care Team can securely access real-time data and patient assessments through the CarePath Platform.
3. To support The Patient’s Care Team in the refinement of an assessment, CarePath AI leverages real time biometrics with patient metadata (medical history, prescribed medication, age, gender, etc.) and daily-updated medical literature (sourced from NIH, NSF, and peer-reviewed published studies).
4. If a potential issue is flagged during assessment, CarePath AI generates follow up questions for the patient and Care Team to utilize in order to further assess the issue.
5. The Care Team can refer to any medical literature upon which CarePath AI based an assessment at any point after the assessment has been performed by following links presented in the assessment.
6. The Patient’s Care Team will make the final determination on a course of care based on their experience and relationship with the patient.
