Theresult is a secure, vendor‐neutral infrastructure that provides just-in-time data to facilitate interventions and reduce readmissions for the chronic condition patient.

    Participation in the TSAP/Community Approach to Aging Well Program resulted in a notable reduction in hospital readmission rates and shortened hospital stays. This significantly enhanced the quality of life for a an often underserved group of dual-eligible patients. TSAP empowered participants to maintain their independence while aging in place.
